What information will the receptionist need to schedule an appointment?

Accepted Answer

Full name, including middle initial, Birth Date, Current phone number,email, and address, The nature or reason for the call/purpose of appointment, Physician’s name, Insurance card (if you have one). You can also book your appointment online.

What should I bring to my appointments?

Accepted Answer

- A valid, government-issued driver’s license or other photo identification.
- Your insurance card (if you have one).
- Your co-pay (if you have one).
- Any prescriptions for medications you are currently taking (bring in your medications or write them down and bring in the complete, up-to-date list).
- Please include your current pharmacy name and address.
- Any X-ray or MRI done before your visit related to your condition or injury (bring in any imaging reports or results)
- If your injury is related to a car accident, personal injury or work injury, bring the following documents; your insurance information (claim or policy No.), police report if any, and if you are represented by an attorney, please provide information.
- Change of address or phone number(s) if you’ve recently moved or made a change.

What if I can’t make my appointment or I need to reschedule?

Accepted Answer

Please call Rand Medical Center or go online to reschedule your appointment as soon as you realize you won’t be able to make your appointment. We understand that situations arise. If you do not notify us within 24 hours of your appointment time, you may be charged for the appointment scheduled. For repeated insufficient notification, we reserve the right to discontinue service.
